copyediting and proofreading are separate stages. or at least they should be. sometimes the former is done during the latter, which can be a mess. sign of cheapness on a publisher's part.

brian, copyediting occurs before the work is set in type, and is the stage where grammar, spelling, punctuation, consistency, and adherence to standard style are all addressed. the editor makes corrections and queries the author and/or production editor to iron out inconsistencies, secure missing info, fix incomplete references, verify facts, and the like. can be light or a virtual rewrite (we call that developmental editing).

proofreading is simpler: the proofer sticks to ensuring that the ms was set in type correctly. you check headings, scan type (sometimes vs. the original manuscript), check page numbering vs table of contents, and give it a final read. if any stray editing/factual problems did survive copyediting, it's sporting to at least flag them for the production editor and/or author. sometimes you can just fix them. can be done on paper or pdf.

brian: not strictly to the formatting, although sometimes modern copyeditors will be asked to apply character or paragraph styles in word, or apply xml coding, so when the ms is typeset, that coding will match the style selections in the publishing software and code text into the proper font, spacing, indent, etc. but copyediting itself doesn't affect formatting. that's the designer's task. but copyediting can affect the holy hell out of the readability and flow of the book.
